Success and Challenges of School Oral Health Program, Kuwait

Abstract:
School Oral Health Program, Kuwait (SOHP) is a comprehensive school-based/linked program providing oral health education, prevention, and treatment to more than 300,000 children in Kuwait Public schools. SOHP had its modest beginning during 1982-83. Since its inception, this program has gone through many administrative and structural changes. Success of SOHP can be measured in terms of oral health outcomes among children receiving services under this. With effective prevention, SOHP has stabilized the caries-levels in permanent teeth and has reduced the unmet needs. SOHP has also faced many challenges over the years which has threatened its existence from time to time. Role of decision makers has been key to sustainability of SOHP. This program will continue to play a very significant role in not just delivery of oral health in Kuwait but also in controlling the burden of oral diseases.

Introduction:
Oral health is the integral part of the overall health of an individual. Children are vulnerable to oral diseases, especially tooth decay (Dental Caries) since all the teeth erupt during their childhood and the newly erupted teeth are susceptible to tooth decay. Also, the oral health habits established in children when they are very young, stays with them for life.
Comprehensive oral health programs for school children plays a major role in controlling the effect of tooth decay among children. Also, school-based programs play a significant role in educating children with healthy eating and oral hygiene habits.
MOH Kuwait introduced School Oral Health Program during 1982-83 to control the burden of oral health diseases among school-aged children.

Obesity and Diabetes: Prevention and Intervention in the Dental Setting

Abstract:

Dental professionals have an important role in the prevention and detection of many oral and systemic diseases because of their diagnostic and screening abilities, as well as the frequency of patient visits. These invaluable skills and practice paradigms should be considered as part of the equation to solve one of the largest public health concerns of our time: the obesity and diabetes epidemic. There is a well-described connection between periodontal disease and diabetes, with implications that the relationship may be bi-directional. Periodontal disease and obesity are associated with inflammatory stress and increased production of proinflammatory cytokines. Clearly, these associations should be reasons for the dental profession to intervene in the rise of obesity. Insufficient sleep is another factor in the obesity problem and screening for sleep habits could be part of a  comprehensive dental assessment, along with height, weight, blood pressure. The dental profession is in a unique position to identify and aid in the treatment of obstructive sleep apnea, a condition associated with obesity and diabetes. The rise of obesity and type 2 diabetes in children is of great concern and the dental profession can play a role in raising awareness of overweight status as well as obesity risk behavior.

Learning Objectives:

  1. Participants will become familiar with the current prevalence of obesity in the US and other countries.
  2. Participants will be able to discuss the relationship between obesity and oral health
  3. Participants will be more aware of their patients’ health risks with respect to obesity.
  4. Participants will be able to implement a screening and possible intervention in their own clinical sites.
  5. Participants will be able to identify behaviors that increase obesity and contribute to poor health.

Introduction:

There is interest in the relationship of body weight and obesity to oral diseases. This interest derives from the recognized association of diabetes and obesity, and the importance of the relationship of diabetes and oral health. The mechanism that is believed to account for the relationship of diabetes and periodontitis is enhanced local and systemic inflammation, and the role of adipocyte and macrophage –associated production of inflammatory mediators is an important part of that relationship. In addition, weight gain is often related to food intake, and the intake of fermentable carbohydrates can have an effect on the dentition and other tissues in the oral cavity. In addition, this association is of interest because being overweight and obese is associated with sleep apnea, and dental professionals are expanding their role in treating this disorder.

Being overweight and obese has also been associated with an increased risk of oral/dental disease. These associations require dental professionals to be vigilant, treat disorders when they are detected, and develop a preventive regimen to reduce the chance of future complications. Further, oral health care professionals need to consider a role in obesity management for their patients. They are well versed in delivering a nutrition message related to carbohydrate intake, and this new responsibility could improve both the oral health and general health of patients seen in dental offices

Topic Description :

Drill and fill to Heal and Seal – A Paradigm in Caries Management

Abstract:

Dental caries is the most common cause of tooth destruction in children. So, early detection and management of incipient lesion, which are present in the initial phase most commonly as whitish spots on enamel surface can prevent loss of tooth structure. There are various methods for the management of incipient lesions; the most effective being use of remineralising agents, use of fluorides and antiplaque agents. Management and prevention of these lesion as early as possible justify the role of a Pediatric and Preventive Dentist hence precluding the severe issues that may arise from tooth destruction.

Introduction:

The early diagnosis and management of carious lesion is the key to the success of the treatments. Different efforts which must be made in order to decrease incidence of caries include identification and monitoring of bacteria, diet analysis and modification, use of topical fluorides and antimicrobial agents with more evidences about various modalities, In 2016, FDI approved a revised version of the Policy Statement on “Minimal Intervention in the Management of Dental Caries” (2002) which recognized that “an operative (‘surgical’) approach should only be used when specifically indicated, e.g., when cavitation is such that the lesion cannot be arrested, or when there are aesthetic or functional requirements”. The aim was to encourage a shift from a restorative approach of dental caries management to the delivery of preventive dental medicine. Yet in spite of this effort, uptake in daily clinical practice so far has been slow. This presentation will help improve our understanding about the use of preventive techniques and newer technologies, and will direct us into different approaches and interventions; overall encouraging fellow ‘Pediatric & Preventive Dentists’ to emphasise more on the latter speciality of our title.

Learning Objectives:

  1. Early detection of the disease process
  2. Management of incipient caries lesion
  3. Decide which types of non-restorative treatments or interventions could be used to arrest or reverse existing non-cavitated lesions in adults and children
  4. Emphasising on new preventive techniques over conventional techniques
  5. Safety, efficiency, feasibility, and effectiveness of SDF in preventing and arresting caries

Topic Description :

Minimally Invasive Treatment of Enamel Defects

Abstract:

Enamel defects are a big problem for many patients. The most famous of them are MIH, Fluorosis, carious white spot, traumatic lesions. The increased demand for aesthetic treatments today necessarily implies greater attention of the clinician in formulating a correct diagnosis and in the management of these defects. The purpose of the webinar is to illustrate how to perform a right diagnosis of these problems, to identify the nature of the lesions and choose the most suitable treatment plan for resolving aesthetic cases. Participants will learn how to do diagnosis of the main enamel lesions and they’ll receive precise minimally invasive treatment protocols to apply in the aesthetic area, including techniques such as home bleaching, chemical microabrasion, resin infiltration, macroabrasion.

Introduction:

Minimally Invasive Dentistry is today a goal for clinicians. A lot of white enamel lesions in aesthetic area can be treat by simple and economic protocols, like resin infiltration. But it’s important that clinician know the difference between superficial and deep resin infiltration, and when it’s necessary apply a “multiphase protocol” with other techniques like dental bleaching or superficial abrasion. In this webinar we’ll show what technique is better for each clinical situation, to always have a good aesthetic result for our patients.

Learning Objectives:

  1. Diagnosis of enamel defect
  2. Minimally invasive treatment protocols
  3. Dental bleaching
  4. Superficial resin infiltration
  5. Deep resin infiltration

Topic Description :

Homecare Plaque Removal in Prevention of Caries and Periodontal Disease

Abstract:

Caries and periodontal diseases are complex diseases caused by an unbalanced relationship between the bacteria in the biofilm and the host defence mechanisms. The influence of the host response is being widely studied but at present times, in the absence of proven means, the control of plaque formation constitutes the basic approach to prevention of both diseases. Plaque must be removed frequently by mechanical self-care methods. Tailored instruction and motivation of a person, regular visits to the dental office with professional feedback and reinforcement seem to be the most successful approach to prevent occurrence, recurrence and disease progression.

Learning Objectives:

  • Discuss as effective plaque control influences prevention and control of caries and periodontal diseases
  • Analyze the parameters of choice for manual and electric toothbrushes choice
  • Evaluate the importance of interproximal devices use and how to choose the most suitable ones
  • Utilize tailored instruction and motivation for each patient

Caries Detection / Diagnosis. A Chimera?

Abstract:

The purpose of this webinar is to discuss the current available methods to detect caries lesions. The current evidence-based caries understanding, based on biological concepts, involves new approaches in caries detection, assessment, and management that should include non-cavitated lesions. Even though the importance of management of non-cavitated (NC) lesions has been recognized since the early 1900s, dental caries has been traditionally detected at the cavitation stage, and their management has focused strongly on operative treatment. Methods of detection of early carious lesions have received significant research attention over the last 20 years. The most common method of caries detection is visual-tactile.

Learning Objectives:

  • summarize the scientific evidence on caries detection and diagnosis – recognize the different caries levels
  • estimate the caries progression rate – illustrate the different caries detection and recording systems
  • predict the feasibility of the new approach
